I certify that I am the owner or representative of this/these pet(s) and I am at least 18 years of age. I hereby grant permission to this establishment to act on my behalf, and in my pet’s best interest, by obtaining veterinary care, at my expense, if deemed necessary, for illness or injury. I further agree to pay for all veterinary and necessary services incurred by my pet during its stay at this facility. This facility agrees to exercise all due and reasonable care to prevent injury or illness to your pet. However, in the event of illness or injury, the owner and employees shall not be held personally liable. We are not responsible for articles left with your pet by you, but will make every attempt to return them in good condition (as your pet allows).

Let it also be clearly understood and agreed that if the pet is afraid or destructive due to stress, aggression, thunder, lightning, gunshot sounds, or other loud noises, that the owner assumes financial responsibility for damages to this facility and/or other pets.

I understand that if I fail to pick up my pet within ten days of notification to my given address, my pet will be considered abandoned and will be handled in accordance with New York state law, and that doing so does not relieve me of my financial obligations.
Proof of protection against external parasites is required. All pets will be checked for external parasites upon admission, if parasites are found, Advantage II will be applied at the owner’s expense. Along with proof of vaccines, a negative fecal sample within the past year is also required. If needed, we will check a fecal sample while staying with us for an additional fee.
